To Register for LPYF (Mid Florida) we must have the following forms / documents:

1. 2010 Mid Florida Physical Form (physical dated this year)

2. 2010 Mid Florida Player Contract / Permission Form

3. Certified Copy of Birth Certificate

4. Copy of Final Report card from 2009-2010 school year

5. Appropriate participation fees paid (see fee schedule below)

All Mid Florida forms on this page are required to be printed Front to Back for proper completion and registration of the participant.
